I got a black hole in Itjtawy. A storage yard collapsed because no architect came around. (Fortunately there was nothing in it to be lost and I save every month anyway.) I immediately cleared away the rubble and built a new one. Then a few weeks later, it became a black hole.
Just to review, a black hole is where each square is replaced by a flat purplish-black square, which you can't clear away or do anything else with. The only way to get rid of it is to take the game back to before the black hole was created, and NOT do whatever you did to create it in the first place.
Before the storage yard became a black hole, it was deformed: if you've ever looked at the crime overlay, you'll see the "tower" of the storage yard is truncated. That's what my storage yard looked like just before it collapsed.
But I did notice one thing about this: I had cleared away the rubble BEFORE the smoke and dust had cleared. When I allowed the smoke and dust to clear out BEFORE removing the rubble, it didn't become a black hole.
