I've found this data for patricians here: http://caesar4.heavengames.com/gameinfo/housingrequirements
But nothing about plebs and equites.

Plebs and Equites don't actually NEED culture coverage, but the city culture rating will be lower if they don't have it. If you want a high city culture rating, then yes, everyone needs access.

Insulas sometimes don't evolve for the lack of entertainment...

It might be lack of variety in food or basic goods. If the plebs have to go too far to the markets, they never seem to get what they need.

Plebs don't need any culture. With one basic good and water from a well they can evolve to medium insula. For evolving to large insula, they need two basic goods and water from a fountain, nothing else.

Eqites also don't really need any culture, too. However, if you don't have at least an odeon, they may leave your city or new emigrants may refuse to come. In this case, the message you'll see (when you click the domus) is "Too little entertainment".

Also, if you don't have at least one Jupiter shrine, he starts to destroy your buildings after one year from the begging of the game had pass.
