It is probably because you caved in to an enemy's extortion. You have to be careful in the later scenarios which requests you fulfill and which you do not, so your kingdom rating does not suffer.

May well be that one.
I do not know or recall the exact messages, but here are some occurrences of the KR falling by 5 points with people pleased (BTW I do not recall anybody mentionning that his/her KR fell because the population itself was unhappy):
-fulfilling an extortion, even if it comes from your Pharaoh himself;
-not paying Pharaoh's tribute (the game checks that you do not have enough money to pay for the tribute, your KR drops by 5 points)
-indebtment at the end of the first year (or any year): if not considered as over-indebtment, it is generally punished with 5 KR points;
-refusing to fulfil a request;
-defeat for the troops sent;
-delaying a request;
-being especially late in your achievements (checking time when 25%, 50% or 75% of the targets required had to be achieved);
-fulfilling a request but being punished for that (in some mad scenarios).
The messages about your Kingdom rating are also sometimes to consider with caution, as two may overlap.

I saw this event, appropriate for a time of civil strife, in Middle Kingdom Thinis. It apparently is an external event ("dissatisfaction in the Kingdom") which has nothing to do with the way the city is governed. If you're also playing Thinis, then there will be other events which raise the Kingdom rating and you can get blessings from Ra.
