On allows for a number of unconnected communities and I had decided to keep housing down to (pre) beer level in about half the settlements. I upped the rest well past mortuary level.
By the time of the endgame, I had a culture rating of 70 and the message lack of mortuary access limits your rating. The Health overseer showed access for 2600 odd people from 4 mortuaries (about 4444 out of 5800 had physician access (9))
As the linen production was also localised I thought well, I'll just add one or two mortuaries in the linen rich area as all I need it to increase my city wide access level.
A year later the message was the same and the overseer told me 2700 odd people now had access from 7 mortuaries (all staffed and having linen)
I fiddled around some more and the message changed to lack of physicians... 4000 with 9.
The overlay showed me half a housing block had lost access, I changed some roadblocks and road layout and the score went back to 4500 by 9 and I got the lack of mortuary message again.
It seems that the access is actually calculated on the houses that can be reached by the health provider, which means creating a "health getto" is not an option.
Entertainment seems to be working somewhat different as my Senet house showed a coverage of 5000 though all it was reaching was half a dozen manors.
I'm not too sure about the other entertainment walkers though....
